Americare At Home, Inc. was founded in 1993 and is a Medicare/Medicaid certified home health care agency with a community of nurses, rehab professionals, and paraprofessionals dedicated to providing the best quality of care to patients at home on a one-to-one basis. We are seeking a qualified per-diem or part-time certified diabetic educator.

Job Duties include:

•Perform comprehensive client assessments and ensure continuity of nursing care
•Ensures timely and accurate communication and documentation of appropriate care
•Clinical training, education and career development of staff
•Promotion of patient, family and/or significant other in the return to optimal physical and mental health through support and education.
•Counseling of clinical staff
•Promotes a safe and efficient working environment for all staff
•Appropriate reporting of incidents, injuries, infection and all other components of risk management.
•Attends and participates in staff meetings, required in-services, and other meetings as requested.
•Observe infection control and standard precautions

Job Requirements

•Massachusetts RN license required.
•Diabetes Educator certification required.

•Current CPR certification required

•Two years of diabetes education experience and/or training, or equivalent combination of education 
  and experience required.

•Home care experience required.

•Wound care experience preferred
•Valid drivers license required
•Reliable transportation required
