Devereux Massachusetts is looking for aFollow AlongProgramCoordinator to join our Clinical department. This position coordinates the provision of intensive home-based family interventions and supports in preparation for, and after, the youth transitions to a community-based placement. The Follow Along Program Coordinator will provide clinical oversight and supervision to achieve the following goals: ensure successful transition of youth to a community-based placement, reduce the length of time youth are away from their home and community, maintain youth in the community and reduce likelihood of recidivism or return to a higher level of care and enhance natural supports and strengthen community connections.

The duties of this position include:

*Providing clinical and programmatic supervision to Follow Along staff and other community-based clinicians as necessary.

*Ensuring that treatment provided is consistent with the respective community-based program model.

*Ensuring that supervisees are able to successfully navigate internal and external systems in a collaborative and consultative manner.

*Providing supervision to assigned case managers and ensuring that they are able to successfully negotiate the internal and external systems to promote quality treatment and discharge planning for clients.

*As necessary, provides direct services to you and families in a manner consistent with professional credentials, training and experience.

Job Requirements

For consideration, an applicant must:

*Possess an advanced degree (M.A., M.S., M.S.W, etc.) in a related social sciences field.

*Maintain an independent license (i.e. LMHC, LICSW, etc.), or be license eligible.

*Have relevant experience providing transitional and/or community-based experience to youth and their families.

*Preference will be given to candidates with previous supervisory experience

*Minimum of one (1) year in providing services to youth and families, preferably the provision of services to youth and families in group home and/or community-based settings.
