Job Description
Tufts Health Plan provides a spectrum of health management programs for members with chronic and/or complex medical conditions. Reporting to the Care Management Team Manager, the Disease Manager Nurse willdemonstrate strengths working independently as well as collaboratively within a team environment. An understanding of the Department business objectives and outcome measures is important to the accountability of the Disease Manager Nurse and the success of the Tufts Health Plan Health Management Program.
The Disease Manager Nurse will provide telephonic intervention to support members with chronic illness including administer a comprehensive disease specific assessment to determine the members specific opportunities for intervention. The goal of the Tufts Health Plan Health Management program is to provide education and health coaching to improve understanding and self management of the individuals chronic disease/condition. Support to the members, both adult and/or pediatric, who participate in the program, will result in decreased unnecessary hospital admissions and decreased emergency room visits and improved disease self management for the individual member.
Job Requirements
Qualifications (Minimum education & experience level required):
Education:
Registered Nurse with current unrestricted state license required. Bachelors in Nursing Required. National Certification in Disease or Case Management is highly desirable.
Experience:
Minimum of five years of clinical nursing experience in various settings with a focus on chronic health conditions
Minimum of 2 years experience required in disease management or chronic condition education specialist. For this position specific experience with heart failure and/or chronic obstructive pulmonary disease is required.
Proficiency in a second language is highly desirable

Working Conditions or Additional requirements (include special requirements, e.g., lifting, travel, overtime)
- Fast paced office environment handling multiple demands.
- Must be able to work 11:00AM to 7:30PM or 11:30AM to 8:00PM up to 1 2 evenings per week as program develops.
- Must be able to exercise appropriate judgment when necessary.
- Must be able to work and communicate with customers in a telephonic office environment
Additional Requirements
Travel required on an as needed basis. Occasional evening, weekend, early morning or extended day work may be required.
